If you would like to request a copy of your clinical records, please fill out the form in its entirety and email it back to us.  Records requests typically take up to 21 business days. 

For records requested on behalf of a current or former client of an Imara Therapist there is a fee associated with producing the records that must be received with the request. Record Requests from external sources must also be accompanied by a signed Request of Information by the client.
